Trimark Announces Wellmark Collaboration on Quality Program — Mar 5, 2010
Trimark Physicians Group and Wellmark Blue Cross and Blue Shield are committed to work with patients to improve and maintain their health status by providing them with the best care possible. This commitment does not end when patients leave the Trimark clinics.
Trimark Physicians Group wants to inform you that we are working with our patients and Wellmark Blue Cross and Blue Shield in a collaborative program that promotes access to care in line with nationally accepted best practices for the treatment of chronic illnesses and preventative care. This program also provides information and supports the patient’s need to improve their health and maintain a healthy lifestyle.
Patients may be contacted by a Trimark staff member or a Wellmark health representative to advise them about services they are eligible for under Wellmark’s plan that may include the following:
- Routine health maintenance services covered by the patient’s health plan (recommended vaccinations, health screenings, etc.)
- Recommended medical testing (such as completion of annual tests for patients with a chronic condition)
- Programs or services available to Wellmark patients that could provide helpful support (health coaching for chronic conditions, wellness programs, stop smoking programs).
- Additional help is also available to Wellmark patients through Wellmark’s Personal Health Assistant 24/7 phone line. Patients can call 1-800-724-9122 any time, day or night, to speak with a registered nurse for health care advice and support.
